Media Release Form

Please note: The Media Release Form should be signed annually by parents when students' work and/or images are going to be used publicly. Please do not place this in the Student Handbook to be a 'blanket acceptance' document. Parents need to read and sign this annually PRIOR to distributing their students' work and/or images on the internet or in other media.
The Media Release Form is not necessary for yearbooks, since the publication of a yearbook is limited to the school community and not public.
